Dr. Steven Rotter has never been interested in guesswork. Not when it involves surgery, not when it involves patient care, and certainly not when it comes to how he runs his practice. Dr. Rotter, the founder of Exclusive Dermatology in West Palm Beach, has built his career around precision, accountability, and a deeply personal approach to care. It’s a philosophy shaped by elite training, relentless standards, extensive experience, and an unwavering belief that patients deserve both honesty and excellence. “I always knew I was going to be a doctor,” Dr. Rotter said. “In fourth grade, I wrote a paper about it. My dad was a pharmacist, and I worked in his store growing up. I knew I didn’t want to be a pharmacist, and the only other thing I could picture was being a doctor.” That early certainty eventually led Dr. Rotter through medical school and into a five-year general surgery residency, where he initially planned to become a plastic surgeon. Midway through that path, though, he discovered a specialty he hadn’t even known existed. “I found out about dermatologic surgery,” he explained. “I switched after two years of general surgery, went to Johns Hopkins for dermatology specifically so I could do a fellowship in dermatologic surgery.” Dr. Rotter opened his first practice upon completing his fellowship, building what became a large, predominantly referral- only surgical dermatology operation. For nearly 30 years, his work focused entirely on skin surgery: cancer, reconstructive, and cosmetic. Most of his patents were referred by other dermatologists, surgeons, and primary care doctors. That referral model demanded more than technical excellence; it required an uncompromising patient experience. “If you have a bad experience with your initial doctor, you don’t go back,” Dr. Rotter explained. “But if you upset a referred patient, the referring doctor may never send you another patient. I couldn’t let patients have a bad experience, from scheduling to healing.” That mentality still defines his practice today. While Dr. Rotter has performed tens of thousands of Mohs, reconstructive, and cosmetic procedures over the course of his career, volume has never been the point. Precision and excellence are. Mohs surgery, one of his specialties, eliminates the guesswork that still exists in many forms of cancer treatment. “When you have skin cancer, what you see on the surface is generally not all there is,” he said. “With Mohs surgery, we don’t guess. I take only what is needed. The skin cancer determines the size and shape of the wound, not the surgeon.” Unlike standard excisions— where less than one percent of tissue margins may be examined—Mohs surgery allows Dr. Rotter to analyze 100% of the tissue margin every time. “Complete margin examination gives Mohs surgery the highest cure rate (approimately 99%) and the smallest wound possible because we don’t take too much or too little. Then reconstruction can safely be done since we know the cancer is actually gone.” After decades of practice, Dr. Rotter moved to Florida and initially retired. That retirement didn’t last, though. In 2024, he opened Exclusive Dermatology, originally envisioning a fee-for-service, insurance-free surgical practice. “‘Exclusive’ isn’t about elitism, it’s about mindset,” Dr. Rotter explained. “Concierge-level service. Saying yes when you can. Treating patients the way you’d want to be treated.” Dr. Rotter’s work spans medical, surgical, and cosmetic dermatology—an intersection he believes requires both scientific mastery and artistic judgment. “There’s an art and a science to this,” he said. “I understand how skin heals, how lasers interact with tissue, how fillers behave.” His goal, he explained, is rejuvenation—not transformation. “You don’t want people to notice you had something done,” he said. “You just want to look better.” Despite being nearly two years into building his new practice, Dr. Rotter shows no signs of slowing down. “I’m excited about growing this,” he said. “I enjoy building.
